LeadForms is a service provided by Artem Gavrikov, individual entrepreneur registered in Georgia. This policy explains what you may not do with the quizzes and forms you build in LeadForms and what we do when someone breaks these rules. It applies to every customer and every quiz, whether it is published on quiz.leadforms.org or on your own domain, and it is part of our terms of service.
1. Data you may not ask for
You may not build quizzes or forms that collect any of the following without a lawful basis:
- passwords and other login credentials;
- payment card details;
- one-time codes sent by text message;
- identity documents or their numbers.
A quiz that imitates a login, payment or verification page to capture that data is phishing, and we always take it down.
2. Impersonation and deception
- posing as another company, a brand or a government agency;
- fraud of any kind, pyramid schemes and “get rich quick” or guaranteed income offers;
- fake or misleading offers designed to obtain personal data or payments.
3. Prohibited content and activities
- illegal goods and services;
- drugs, and prescription medicines without the required license;
- weapons;
- gambling and betting without a license;
- adult content;
- any form of child exploitation;
- content that promotes hatred or violence.
4. Spam and technical abuse
- sending spam or bulk messages to people who have not given their consent;
- distributing malicious code through a quiz;
- trying to get around the security measures or limits of the service;
- overloading the service on purpose or with automated traffic.
5. Third-party rights
You may not publish text, images, logos or other material that infringes someone else’s copyright or trademarks.
6. Minors and sensitive data
- You may not collect data from minors without the consent of a parent or legal guardian.
- You may not collect sensitive data (health information and other special categories) without an appropriate lawful basis and the person’s explicit consent.
7. Your responsibilities as a customer
- Have your own privacy policy and show it in the quiz: people who leave their details need to know who receives them and why.
- Have a lawful basis for collecting and using leads, such as the person’s consent.
- Follow the policies of the platforms where you advertise the quiz, such as Google, Meta and TikTok.
For your leads’ data, you are the controller and we are the processor. The details are in our Data Processing Addendum.
8. What we do when the rules are broken
Depending on how serious the case is, and without prior notice, we may:
- unpublish the quiz;
- suspend your account;
- delete your account;
- report the matter to the competent authorities.
If an account is blocked for breaking this policy, payments for the blocked period are not refunded. See our refund policy for details.
9. How to report a quiz
If you see a quiz built with LeadForms that breaks these rules, email info@leadforms.org with the subject line “Abuse”. Include the link to the quiz and tell us what is wrong. You don’t need to be a customer to file a report.
We review every report within 2 business days. Phishing and fraud get priority.
10. Changes to this policy
We may update this policy. The current version is always published on this page with its update date.
