What it is for
The form opens on top of the page, so visitors do not lose what they were reading. They fill in two fields and go back to the site.
Callback requests
A “Request a call” button in the corner of the screen, or a click on the phone number in the header, opens the form. Your sales rep calls back.
Bookings
A consultation, an on-site measurement, a trial class. A date field and a list of services get the appointment booked without endless back-and-forth.
Quote requests
“Get a quote in 15 minutes” is a clear reason to leave a phone number. You can add a couple of questions about what the customer needs.
What the form can do
You set it up in the editor, no code. The preview on a website mockup shows every change instantly.
Custom fields
Name, phone, email, free text, dropdown list, single choice, checkboxes and date. Any field can be marked as required.
Phone mask
The number is formatted for the country as it is typed and checked as soon as the visitor leaves the field. Errors show up before submitting, not after.
Data consent
As text below the button or as a checkbox, with links to your documents. The dashboard will not publish the form without a link to your privacy policy.
Marketing opt-in
A separate, optional checkbox for consent to marketing messages. The answer comes with the lead, so you know who you can send offers to.
Thank-you screen
Your own text after submission and a button, for example “Message us on WhatsApp” or “View the catalog”.
Design
Image on the left, on top or no image, a label above the headline and a note below the button: “We call back within 5 minutes during business hours”.
Where the leads go
A form lead works just like a quiz lead, so everything you already set up for your quizzes works here too. It is billed the same way: according to your plan, like a quiz lead.
Email, WhatsApp and CRM
Notifications arrive as soon as the form is submitted, and a deal is created in your CRM automatically. For example, with the Kommo integration, as well as Bitrix24 and webhooks.
Where the customer came from
The UTM tags of the visit come with the contact details. You see which ad brings leads and which only brings impressions.
Form analytics
Views, clicks and leads by day, conversion, and a mobile vs. desktop breakdown. A/B testing of copy and design is included.
Pop-up form or quiz?
Both bring leads, but they fit different situations. You can combine them: the widget button opens the quiz on top of the page.
You only need the contact
The visitor already knows what they want: to book, to get a call, to know the price. A two-field form is enough.
You need to understand the case
The price depends on several factors and your rep needs details. Then a questionnaire with questions or a quote calculator is a better fit.
The visitor is still choosing
A quiz helps them decide and leads them to leave their details through questions. You build it in the quiz maker and open it from any widget.
When and to whom to show it
The form can open on its own, with the corner button, or with a click on any button on your site. You set the rules in the editor.
At the right moment
A few seconds after the page opens, at a set scroll depth, when the visitor tries to leave, after a pause in activity, or not before the second or third page. Or only when someone clicks the side tab or an element on your website.
No endless repeats
On every page, once per visit, or at most once every N minutes. The widget stops showing once the visitor clicks the button, submits a request, or after the number of views you choose.
Only to your audience
Pages you pick by URL, visits from a specific campaign by UTM tags, mobile only or desktop only. A campaign offer stays hidden from everyone else.
On a schedule
Start and end dates of the promotion, hours and days of the week in the visitor’s time zone. A weekend promo turns on and off by itself.
How to add it to your website
No developer needed. You paste the code once, then turn widgets on and edit them in your LeadForms dashboard. Step-by-step instructions for each platform are in our guides.
Create the widget
Open the Widgets section in your dashboard and pick a type. Text and colors come prefilled with an example, so you only adapt them to your business.
Check the preview
The editor shows the widget on a website mockup and applies changes instantly. The “As on the site” mode lets you check when it will appear.
Paste the code once
One code for all the widgets in your account. If a LeadForms quiz is already on your website, there is nothing to paste: widgets run on the same script. The dashboard checks on its own that the code is in place.
Where it works
On any website where you can paste code, including through Google Tag Manager.
More widgets for your website
All widgets install with the same code. The button in any of them can open your form or your quiz.
