A survey nobody reads is useless. This one has three specific jobs, and in all three you know who answered.

A service rating after every order or visit. You spot a decline early, before it shows up in sales.

An NPS-style score: “On a scale of 0 to 10, how likely are you to recommend us?” It separates happy customers from unhappy ones.

How to get people to answer
Three decisions matter more than the design.
Five steps at most
A long survey gets abandoned halfway. If a question will not change anything you do, cut it.
Send it on time
Ideally the same day as the delivery or visit, while the customer still remembers how they were treated.
Act on the answers
Before launch, decide who reads complaints and how fast they respond. A survey with no follow-up annoys people more than not asking.
