Accept the invite
Open the LeadForms invite on Zapier and sign in to your Zapier account (you can create one for free). Click “Accept Invite & Build a Zap”: LeadForms will appear in your list of Zapier apps.
Copy your LeadForms key
In your LeadForms dashboard, open “AI agents and API” → the “Zapier / Make / n8n” tab and click “Create key”. Copy the key: you will need it in the next step. Don’t share it with anyone.
Create a Zap and choose the trigger
In Zapier, click “Create” → “Zap”. For the trigger, search for LeadForms and choose the “New Lead” event. When Zapier asks you to connect an account, paste the key from step 2. Click “Test trigger”: Zapier will show a sample lead. If you don’t have any leads yet, fill out your quiz once and test again.
Choose where leads go
Add an action: for example, a new row in Google Sheets, an email in Gmail or a message in Slack. Map the lead fields (name, phone, email, answers, UTM) to the app’s fields, run a test and click “Publish”. Done: every new lead is now sent automatically.

The integration is free in LeadForms and works on any plan. Zapier has a free plan with a monthly task limit; if you send a lot of leads, check the plans on Zapier’s website.
Accept the invite from step 1 first, using the same Zapier account you build the Zap in. Once you accept it, LeadForms shows up in the app search.
No. The Zap sends leads that arrive after you turn it on. You can download earlier leads from your dashboard.
Turn off or delete the Zap in Zapier. To revoke access completely, open “AI agents and API” in your LeadForms dashboard and revoke the key.
Use webhooks: they send leads straight to your own system, Make or n8n.
