Notification settings are on the “Notifications and settings” tab: pick the email and click “Edit”.

Open your LeadForms dashboard, go to the “Notifications and settings” tab, then “General settings”, click “Edit” and change the name. By default, the quiz name is taken from its headline.
Everything about design and color is on the “Form and questions” tab: the color picker is in the panel on the right.
Once you pay the outstanding balance, open your quizzes page and click the “Leads” button for that quiz.
Go to the “Quiz installation” tab and choose “As a block on the website”.
