How it works
Clients come in for lash extensions with a photo from social media, and the main question is what exactly they want. The quiz turns that picture into a booking. First, whether old extensions need to be removed: it's a separate job that takes its own time, and you need to know about it in advance. Then the style (classic, volume or “I'd like the artist's advice”) and the color: black, brown or white. The client attaches a photo of the look she wants as a file, and the artist understands her expectations without a long back-and-forth.
The adhesive allergy question is the most important one in practice. The “yes”, “no” and “not sure” answers segment the leads: clients who aren't sure should be offered a patch test. There's no need to collect detailed health information in the quiz: that's a conversation for the appointment. If an answer mentions a reaction to adhesive, it's better to let the client know the procedure is done after a consultation.
As a result, the quiz shows the price of the procedure with a discount (30% in the original template) and then offers to settle on a date and time. The template description names a specific city: if you work somewhere else, change it. Keep only a discount you actually offer: in beauty, distrust of inflated “discounts” runs especially high, and an honest price with a small bonus works better.
With this template you can make a quiz in LeadForms: change the questions, images and offer for your business in about 10 minutes, no developer needed.
