How it works
The quiz is short, five questions, and that's the right call: parents are comparing several camps and won't fill out a long form. The child's age is set with a slider; hobbies (reading, sports, drawing, singing, dancing, games) help suggest the right session or activities, and the length of stay in days sets the price.
The template's math is simple: the number of days times a daily rate. The result description lists what the stay includes: lodging, meals, activities, supervision by counselors and medical staff. Set your own daily rate in the “Results” step; if you run fixed sessions rather than flexible dates, it's better to replace the slider with session options, each with its own price.
The question about allergies and medical restrictions is necessary: the camp is responsible for the child. But this is health data, a sensitive category: depending on your local regulations, the form may need a separate consent, and the details are better collected on a medical form rather than in the quiz. The template's discount grows as the parent goes through the quiz; spell out in the result which sessions it applies to.
