A request for a dog sitter is almost always urgent: work starts tomorrow, vacation is in a week. At the same time, it's emotionally hard, because the owner is handing their dog to a stranger. The quiz helps with both: it's fast and asks up front about what worries the owner: the dog's size, whether they need boarding or just walks and how many times a day the dog needs to go out.
The most important question is about special needs: whether the dog needs medication or a special diet, or doesn't get along with other animals. That's what makes arrangements fall apart when it comes up late. The open field for describing the pet is also there for a reason: owners write things in it that genuinely help choose the right sitter.
Separating services (walks, day care, multi-day boarding) means separating different prices and different sitter workloads, and you need to see that in the request right away. The dog's address at the end settles the geography: sitters are matched by area, and without an address the search makes no sense. In the result, show two or three candidates, not one: the owner should feel that they made the choice.
