People look for a repair shop in "something's knocking in the suspension, it's urgent" mode. At that moment a long form loses: the person goes with whoever is easiest to book. That's why the template is short: car make, what's bothering them, and whether there's a warranty. Three steps and the request is already with the mechanic.
The symptom list is written in driver language, not mechanic language: a strange squeal, knocking in the front end, electronics acting up, there was an accident, scheduled maintenance, "I want to make sure everything's fine." The last two items matter as much as the breakdowns: they're the preventive work that keeps the shop's workload steady between accidents and peak season.
The factory warranty question solves a practical issue: a car under warranty has different maintenance logic, OEM parts, and stamps in the service book. Knowing this before the car arrives helps both sides. And in a quiz like this there's no need to show a price: nobody knows it before diagnosis, and trying to guess ends in an argument next to the lift.
