How it works
In food delivery, an unhappy customer almost never calls: they just don't order again or they leave a harsh review. A survey right after the order gives them a way to tell you instead of the whole internet. The questions follow the delivery chain: how easy it was to order, whether it arrived on time, whether it arrived intact, how the courier behaved. That shows where the process breaks: in the app, in the kitchen or on the last mile.
The photo upload is a separate strength of the template. If the order arrived damaged, the customer attaches a photo right in the survey, and you don't have to ask for it in a chat. The question asking what went wrong with ordering appears only to those who reported a problem: you set this up with branching, and the survey stays short for satisfied customers.
What matters most in a survey like this is response speed. If a negative answer comes in, a manager should message the customer the same day: apologize and offer a fix. Connecting notifications to WhatsApp or Telegram helps with that. Each completed survey counts as a lead. The template's coupon makes sense as a thank-you for responding, but don't promise it only in exchange for a good rating: that skews the results.
This template is built in the LeadForms survey and questionnaire maker: questions, answer options and form fields adapt to your task, and responses arrive in your email, Telegram and CRM.
