How it works
An unhappy patient rarely says so at the clinic: they write a review on the maps. A post-visit survey gives them a chance to speak up earlier, and to you rather than the whole internet. The template asks about what a patient can actually rate: how fast the checkup went and how long they waited, how the doctors and staff did, and whether the results were explained clearly.
The question "Did you get enough information about your health?" matters more than it seems. The answer "I still have questions" signals that the patient is leaving worried and may not come back for a follow-up. Review these answers separately and call back. The website question, by the way, shows where patients can't find information about procedures.
The survey is about the service, not about health: don't collect diagnoses, symptoms or treatments there, since they are sensitive data with their own requirements. And be careful with rewards: the coupon in the template works as a thank-you for completing the survey, but it can't be used to reward positive reviews on the maps. That breaks platform rules and damages trust.
