How it works
An internal survey only works when employees trust that their answers won't be used against them. That's why it's built on scales and ready-made options instead of open questions like “what don't you like”: rating team communication on a scale is easier and safer than filing a complaint. There are only two free-text fields: one about computer equipment problems and one about what should be improved.
The template is designed for remote and hybrid work: the lack of in-person contact, slow information sharing, the blurred line between work and rest. A separate block covers IT support and equipment: it's the most common everyday frustration in remote work and also the easiest to fix. Questions about planning the workday reveal what people rarely say out loud: unclear tasks, sudden distractions, the habit of putting everything off.
Decide on anonymity up front and be honest about it. If the survey is anonymous, the contact form fields should be optional or not used at all. A question about tenure in a small team can identify a person: use wide ranges. Most important, agree in advance on what will be done with the results and tell the team. A second survey after a first one that changed nothing won't get honest answers.
