How it works
A guest who didn't like something almost never tells the server: they just don't come back, or they leave a review on maps. A survey with a QR code on the table or on the receipt gives them a third option: telling you. The template asks about the essentials (service on a five-point scale, speed, food) and has separate fields for awkward moments and menu suggestions.
For a chain, the template includes a city selection and a separate list of addresses for each city; show those questions conditionally, with branching, so the guest only sees the addresses in their city. That turns the survey into a tool for comparing locations: within a month you can see where people always complain about speed and where about the food. The likelihood-to-recommend score, from 1 to 10, is easy to track for each address separately.
Two things are worth getting right. A free treat is a good reward for completing the survey, but not for a positive review on maps: that kind of incentive breaks platform rules. And the promotions opt-in: in the template it is a separate question with explicit consent, and that is how it should be, because marketing messages can only go to people who agreed to them, so the opt-in is never checked by default.
